Summer Employment Documents and Training

New Hires

Please complete and submit the following documents to the camp coordinator for payroll processing.

Consent Release and Authorization for a Background Check – Must be completed and cleared before the first day of work.
New Hire Employment Form
W-4 - Federal Tax Withholding Election
K-4 - Kentucky State Tax Withholding Election
I-9 - Employment Eligibility Verification
Drug-Free Workplace Form
Direct Deposit Form
Summer Camp Guidelines and Expectations Form - Related document for review For Camp Counselors Brochure
Preventing Sexual Harassment Training - Online training program to be completed on or before the first day of employment. An email confirmation of training completion will be sent to the coordinator.

Returning Summer Camp Workers (or those who have previously worked at Transy)

Please complete and submit the following documents to the camp coordinator.

Consent Release and Authorization for a Background Check – Must be completed and cleared before the first day of work.
Required every year for working with a summer camp.
Summer Camp Guidelines and Expectations Form - Related document for review For Camp Counselors Brochure
A copy of the signed form must be on file with the camp coordinator.
Preventing Sexual Harassment Training – Must be completed once. Not necessary if it was completed during a previous summer or in relation to a prior work assignment at Transy. Online training program to be completed on or before the first day of employment. An email confirmation of training completion will be sent to the coordinator.
